To change your beneficiary you must complete the Change and Service Request Form. Please note that the form must be signed and dated in the presence of a notary. You may also call our Customer Service department to obtain a copy of the form at (800) 626-1899.

Be sure to complete the Change of Beneficiary, Update Contact Information and Policyowner Authorization sections. You may fax the completed form to Customer Service at (919) 313-7931 or mail form to:

How do I file a death claim?

You may call us at 1 (800) 626-1899 to report the claim and request a death claim form. In order to process the claim, we need the following information:

Death Claimant’s Statement, signed by beneficiary and
Certified Death Certificate
You may need further documentation for your claim. Paperwork can be returned to us at:
